Can I leave the backup withholding part blank on bank application?

Ok I opened a college combo account online with Wells Fargo. I received the "Consumer Account Application" and there is a part that I do not understand. "Request for Taxpayer Identification Number and Certification (Substitute Form W-9)". What do I do? Do I: 1.) Leave it blank 2.) Check the "subject to backup withholding" box or 3.) Check the "exempt from backup withholding"? I have no idea. I haven't paid taxes before or anything.
